What is ACACIA?
Acacia was developed to provide intensive outreach support to people who have extensive histories of diagnosed mental health issues, complex needs and homelessness. Acacia operates in accordance with the Psychosocial Rehabilitation Model.
The consumer determines what areas of their life they want to work on. In consultation with the worker, a plan for achieving this is developed. Workers act as a resource providing information, support, advocacy and problem solving. Workers can also “fast track” consumers to other Western Region Health Centre services.
Typically, support is provided for a period of two years, depending on the needs of the client. The workers have caseloads that enable them greater flexibility in responding quickly to presenting needs of clients. Presenting issues might include accommodation, mental and general health, alcohol and other drugs, legal, financial, etc.
Eligibility
To be eligible for Acacia consumers need to:
• Have a diagnosed mental illness or history
of involvement with clinical mental health services;
• Have an extensive history of homelessness;
• Have complex needs (i.e. alcohol & other drugs, general health issues, legal, financial, etc.);
• Be over 16 years of age;
• Be living in or have connection with the City of Maribyrnong or Hobson’s Bay.
Generally, clients of Acacia have experienced
numerous barriers to mainstream services often as a result of psychiatric issues and/or challenging behaviours.

CONSUMER CONSULTATION
Consumer Consultation is highly valued by the Western Region Health Centre. Acacia was formerly known as the Intensive Outreach Program. In October 2005, staff conducted surveys and held a focus group to develop a new name. The name Acacia was born out of this process.

The Centre is conveniently located in Paisley Street, close to Footscray and Middle Footscray stations, tram and bus routes. Parking is also available nearby.
