What is the Living Well Program?

The Living Well Program helps people with chronic health conditions to live a healthy life. It also helps those at risk of developing chronic health conditions.

What is a chronic health condition?

A chronic health condition is any illness that is long term. Asthma, diabetes and heart disease are all examples of chronic conditions, but there are many others.

What does the Living Well team do?

Having a chronic condition can impact on your daily life. Work, exercise, socialising, or just taking care of yourself, can all be affected.
The Living Well Team member (health coach) works with you individually, your family, carer, doctor and other health professionals to manage your condition and achieve the best possible health outcomes.

 

 

The Living Well team can help you to:

• Learn more about your condition
• Learn ways to take care of yourself (exercise, healthy eating, taking your medication, going to the doctor, quitting smoking)
• Know what to do when symptoms get worse
• Plan ways to help you do your normal activities (go to work, meet with friends)
• Plan goals to help you improve your health
• Make good health choices so you have more control over your condition
• Increase your confidence in managing your condition

 

 

Referrals

Who is Eligible?

The Living Well program is available to adults living, working or studying in Brimbank and Maribyrnong, who have one of the following newly-diagnosed or early stage:
• Asthma
• Chronic obstructive pulmonary disease (COPD)
• Heart Disease
• Diabetes
• Arthritis

Or two of the following risk factors:

• Poor Diet
• Physical inactivity
• Smoking
• Obesity
• High blood pressure
• High blood sugar/pre-diabetes

Appointments

To make an appointment, contact ISIS Primary Care (Brimbank) or Western Region Health Centre (Maribyrnong), or ask your GP to refer you to the Living Well team.

Interpreters

Interpreters are available upon request.

There is no cost for the Living Well Program
